By providing your personal information you are consenting to Coloplast Pty Ltd (Level 4, 1 Acacia Place, Ferntree Business Park, Notting Hill, Victoria) using your personal information for administrative purposes and to process your sample request. From time to time, we let our customers know of new products or services which may be of interest by phone, email, or post. If you have consented to the collection and use of your personal information by Coloplast to provide you with information regarding new products or services, you consent to us contacting you in this way. The information we collect from you is strictly confidential. Coloplast will not reveal, disclose, sell, distribute, share or pass that information on to any third parties outside of Coloplast’s control. We do allow your information to be provided to other select organisations who perform certain operations for us. We bind these companies to the same standard of care as we do ourselves so they cannot provide this information to anyone else. We enforce this through contractual arrangements. We will only use your personal information for any purpose which is related to the products or services we provide to you, or for a purpose for which you would reasonably expect us to use the information eg. marketing, product development, research etc. If we wish to use any of this personal information or data for any other purpose, we will not do so unless we have your expressed content to do so. You may, at any time, express a wish not to receive any marketing communication from us.
Coloplast provides a health service. We will only disclose health information about you with your consent in writing.
We may disclose your personal information to our offices in Poland to process your sample request, but we will ensure that your personal information will be used and disclosed in accordance with the Australian privacy laws. Our privacy policy contains information about how you may access and seek correction of the personal information that is held by us, how you may complain about a breach of your privacy and how we will deal with such a complaint. You can contact Coloplast on 1800 653 317 if you have any questions about how your personal information will be handled or if you would like to receive a copy of our privacy policy. This promotion is not affiliated with the National Disability Insurance Agency (NDIA), nor is it an NDIA-sponsored promotion. Neither Coloplast or Mogo Wheelchairs will be liable in any way for any breach of the terms of your NDIS plan under any circumstances.
